Job Title: Nurse Practitioner - Home-Based Medical Care

Optum Home & Community Care is seeking a highly skilled Nurse Practitioner to join our team in the Saint Louis, MO area. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in their homes.

Job Summary:

We are looking for a dedicated and compassionate Nurse Practitioner to work with our team in providing comprehensive, coordinated care to patients with multiple chronic conditions. The ideal candidate will have a strong background in primary care, home-based medical care, geriatrics, or palliative care, and will be able to work effectively in a fast-paced, dynamic environment.

Responsibilities:
  • Provide comprehensive assessments and develop individualized care plans for patients with complex, chronic conditions
  • Collaborate with the interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals, to ensure seamless care coordination
  • Prescribe medications and order diagnostic tests as needed
  • Provide education and support to patients and their families on disease management and self-care
  • Participate in ongoing quality improvement initiatives to ensure the highest standards of care
Requirements:
  • Completion of a Nurse Practitioner accredited program
  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Current, unrestricted RN licensure in the State of Practice
  • Current, unrestricted NP licensure in the State of Practice
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
  • Access to reliable transportation
Preferred Qualifications:
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of CPT, ICD-10, and HCPC codes
  • Knowledge of clinical standards of care
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
