Nurse Practitioner

The Home Care Registered Nurse (RN) plays a vital role in planning, organizing, and directing home care services. With a focus on community health education and experience, this RN builds on the resources of the community to create personalized care plans that meet the unique needs of individuals and families within their homes and communities.
Key Responsibilities:
• Conduct thorough home health assessments to determine patient needs and develop comprehensive care plans.
• Provide skilled interventions, including patient care, teaching, and other necessary services as ordered by the physician.
• Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan in collaboration with the Case Manager.
• Use health assessment data to determine nursing diagnoses and develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions.
• Initiate appropriate preventative and rehabilitative nursing procedures, administer medications and treatments as prescribed by the physician, and counsel patients and families in meeting nursing and related needs.
• Identify discharge planning needs during nursing visits, communicate needs to the team as part of the care plan development, and assist in implementing prior to discharge of the patient.
Requirements:
• Minimum of two years' experience in public health, home care, or hospice nursing, with at least one year in the area of public health or home care.
• Graduation from an accredited school of nursing is required.
• Current licensure to practice professional nursing in New York State and CPR certification are necessary.
