Job Summary:

The Registered Dietitian (RD) is a key member of our healthcare team, responsible for providing personalized nutritional care plans for older adults. As a part-time dietitian, you will work collaboratively with our interdisciplinary team to assess and address the nutritional needs of our participants.

Key Responsibilities:

  • Conduct comprehensive nutritional assessments and reassessments to identify individualized care needs
  • Develop and implement personalized nutrition care plans to promote optimal health and well-being
  • Provide education and support to participants and caregivers on healthy eating habits and nutrition-related topics
  • Collaborate with the healthcare team to develop and implement care plans that address the physical, emotional, and social needs of our participants
  • Document changes in medical records and participate in care planning meetings
  • Assist in monitoring meals and snacks to ensure they meet the nutritional needs of our participants
  • Coordinate with other healthcare professionals to ensure seamless care delivery

Requirements:

  • Bachelor's or master's degree in Nutrition or a related field from an accredited program
  • Completed Dietetic Internship and passed the Commission on Dietetic Registration (CDR) exam
  • Current Michigan Registered Dietitian (RD) status or eligibility to sit for the CDR exam
  • CPR certification or willingness to obtain within 90 days
  • Master's degree plus approved work experience preferred
  • Certified Diabetes Educator (CDE) status preferred
  • Minimum of one year of experience working with the frail and elderly
