Job Details

We are seeking a skilled LPN to provide high-quality patient care in a long-term care setting.

Key Responsibilities:

  • Administer medications and treatments as prescribed, while observing patients for any adverse reactions.
  • Monitor and report changes in patients' conditions to the interdisciplinary nursing team.
  • Assist with the development and implementation of individualized care plans.
  • Document patient information accurately and comprehensively.
  • Maintain patient confidentiality and adhere to HIPAA regulations.
  • Act as liaison and maintain positive relations with residents and their families and physicians.
  • Contributes to/modifies the patient care plan.
  • Organizes/prioritizes routine and non-routine nursing care activities.
  • Adheres to standards of practice and safe technique in preparation and administration of medications.
  • Develops and maintains effective relationships through effective and timely communication.
