Nurse Care Specialist Job Summary

This role involves delivering top-notch patient care in a long-term care setting. The ideal candidate will have experience in administering direct patient care, performing assessments, and collaborating with healthcare teams.

Main Responsibilities

  • Medication administration and treatment support as prescribed by physicians.
  • Monitoring and documenting patients' vital signs.
  • Supporting daily living activities, such as bathing, dressing, and feeding.
  • Conducting regular health assessments and reporting changes to supervisors.
  • Providing wound care and other clinical procedures as required.
  • Ensuring safe, timely, and respectful patient care delivery.

Additional Duties

  • Accurate documentation of patient care in EHR or paper charts.
  • Maintenance of up-to-date care plans.
  • Prompt documentation of incidents or changes in patient conditions.

Education and Experience

  • Degree from an accredited Practical Nursing program.
  • Current state licensure as a Licensed Practical Nurse (LPN).

Estimated Salary: $60,000 - $70,000 per year.

Location: Yuma, AZ 85364.

Shift: 6:00 AM - 6:00 PM.
