The Clinical Pharmacist plays a vital role in improving the health outcomes of Virginia Garcia Memorial Health Center patients. This position collaborates with primary care providers and staff to deliver high-quality pharmacy services, including drug therapy management, patient education, and medication reviews.
Key Responsibilities- Provide collaborative drug therapy management services to patients in designated clinics.
- Assess patients, adjust medications, screen for drug interactions and adverse effects, and provide disease/medication education.
- Provide patient education one-on-one during clinic visits, at group visits, or as part of disease-specific initiatives.
- Collaborate with medical staff to provide education and training on medication use and management.
- Act as a drug therapy resource to providers and other members of the healthcare team.
- Participate in multidisciplinary teams focused on outcomes research and disease management projects.
- Registered Pharmacist, licensed in good standing with the State of Oregon.
- Preceptor registration.
- Residency training (PGY1) or comparable ambulatory care clinical pharmacy experience in a community pharmacy setting.
- Advanced training in Ambulatory Care (PGY2) and Chronic Disease Management desired.
The Clinical Pharmacist will work in a well-lighted, ventilated environment, with possible indirect exposure to blood-borne pathogens and potentially hazardous chemicals.
