Job Title: Physical Therapist - Home Health

We are seeking a skilled Physical Therapist to join our team at Highlands Home Health, a part of LHC Group. As a Physical Therapist, you will play a vital role in helping patients achieve their maximum potential and improve their quality of life.

About the Role:
  • Evaluate patient care needs and develop personalized treatment plans to address functional status, mobility, endurance, and pain management.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Provide expert physical therapy interventions to enhance patient well-being and promote independence.
  • Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
Requirements:
  • Current Physical Therapy licensure in the state of AL.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
