Position Overview

The Care Coordinator at Clarvida - Tennessee is responsible for delivering approved therapeutic interventions, managing case coordination, and facilitating resource connections for clients and their support systems, including family members, legal representatives, and other stakeholders. The goal is to implement necessary changes for individuals and families in need.

Key Responsibilities

  • Conducting comprehensive assessments during client intake, performing community visits, providing transportation, and fulfilling on-call responsibilities while delivering approved therapeutic services such as coping strategies, behavior modification techniques, parenting education, and crisis management.
  • Collaborating with the treatment team assigned to each client to develop, customize, and regularly update Treatment Plans and related documentation, ensuring compliance with organizational policies, state regulations, and accreditation standards.
  • Acting as a liaison and advocate for clients, engaging with other professionals and agencies involved in their care.
  • Participating in supervision and treatment team meetings, presenting updates on assigned clients as required.
  • Supporting colleagues to achieve optimal outcomes for all clients and their families.
  • Completing clinical documentation within designated timeframes and addressing any deficiencies promptly.
  • Maintaining high standards of quality by conducting peer audits as assigned and contributing to the Performance Quality Improvement Cycle, utilizing data to enhance client and program services.

Qualifications

  • Bachelor's degree in a Human Services discipline from an accredited institution.
  • A minimum of one year of relevant experience in the mental health sector.
  • Awareness and sensitivity to diverse cultures and socioeconomic backgrounds.
  • Comprehensive understanding of child development and behavior modification principles.

Preferred Qualifications

  • Experience working with children and adolescents in a therapeutic, community-based treatment setting.
