Job Summary:

The Licensed Practical Nurse (LPN) will provide skilled nursing care to patients in the home setting, under the supervision of a Registered Nurse. This role requires a strong clinical background, excellent communication skills, and a commitment to delivering high-quality patient care.

Responsibilities:
  • Provide and document skilled nursing care in accordance with physician orders and developed care plans.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan to ensure effective patient outcomes.
  • Take appropriate nursing action based on assessment and achieve expected outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
