Job Summary

Mass General Brigham Healthcare at Home is seeking a skilled Registered Nurse to provide high-quality clinical services to patients in various settings within the community. As a key member of our interdisciplinary team, you will deliver patient care that aligns with our organization's philosophy, policy, and standards of community health practice.

The ideal candidate will possess a strong background in acute care nursing, with a minimum of one year of experience. Prior home care experience is preferred, but not required. You will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families.

Responsibilities
  • Provide high-quality, patient-centered care in various community settings
  • Assess patients' needs and develop individualized care plans
  • Implement care plans and provide education and support to patients and their families
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Document patient information accurately and thoroughly
Requirements
  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one year previous nursing experience in acute care setting
  • Prior home care experience preferred
  • Ability to work independently and as part of an interdisciplinary team
  • Strong communication and interpersonal skills
