Job Description
We're seeking a skilled Patient Care Coordinator to join our team in Phoenix-Estrella, AZ. In this role, you'll be responsible for helping patients achieve optimal outcomes using TMS treatment. This includes providing personalized coaching and support, as well as collaborating with cross-functional teams to ensure seamless patient care.
Responsibilities
- Develop and implement customized treatment plans for patients.
- Provide education and support to patients throughout their treatment journey.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Meet and exceed performance metrics and goals.
Requirements
- High School Diploma/GED required.
- Minimum 3 years' experience in healthcare or a related field.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment.
Compensation
- Salary range: $45,000 - $55,000 per year (estimated).
- Comprehensive insurance benefits (Medical, Dental, Vision) and 401K.
- 20 days off annually (10 PTO days and 10 Holidays).
