We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of our patients.
Responsibilities- Perform home visits and complete coordination of client care
- Oversee the frequency of visits for the episode and consult with physicians as needed
- Accurately collect OASIS data and ensure medication profiles are up-to-date
- Attend weekly case conferences and monthly case manager meetings
- Current RN licensure in the state of employment
- Minimum one year of clinical experience preferred
- Demonstrate knowledge and skill in current nursing practice
- Valid state driver's license and automobile liability insurance
