Inpatient Social Worker

Grand View Hospital is seeking a highly skilled Inpatient Social Worker to join our team. As a key member of our healthcare team, the successful candidate will provide supportive and informational counseling and discharge planning to adult, pediatric, adolescent, and geriatric patients and their families.
Key Responsibilities- Provide consultation to hospital staff, community agencies, and other individuals to resolve patient problems.
- Facilitate support groups to discuss concerns related to specific diseases or aspects of care, as per established Grand View Health protocol.
- Obtain and update information on community resources to provide patients, families, and staff with accurate information.
- Participate in interdisciplinary team meetings to identify patients in need of social work intervention and enhance patient care and communication among the healthcare team.
- Develop and maintain good working relationships with community agencies and reach out to the community to develop new resources to meet patient needs.
- Participate in community planning activities related to patient interests and Grand View Health.
- Work collaboratively with other healthcare disciplines to facilitate discharge planning and care coordination to meet patient and organizational needs.
- Intervene with physicians and ancillary departments concerning clinical and utilization issues to ensure optimal patient outcomes.
- Conduct psychosocial and discharge planning assessments within one working day and perform reassessments per departmental policy.
- Participate in Quality Management projects to ensure appropriate Case Management intervention and provide a mechanism for problem resolution.
- Arrange community services prior to discharge to meet patients' post-discharge needs, recognizing and documenting patient choice of service providers.
- Bachelor's of Social Work, required.
- 1 year of hospital inpatient social work experience highly preferred.
- Licensed Clinical Social Worker, preferred.
