Trinity Health PACE is seeking an experienced Nurse Practitioner to lead our geriatric care team. This role requires a highly skilled professional with expertise in primary care, leadership, and education.
Responsibilities- Leadership: Provide strategic direction for the geriatric care team, ensuring alignment with Trinity Health PACE's values and mission.
- Primary Care: Deliver high-quality primary care services to participants, including health assessments, treatment plans, and medication management.
- Education: Develop and implement educational programs for staff and participants on various health topics, including chronic disease management and preventive care.
- Clinical Governance: Collaborate with the Medical Director and Clinic Supervisor to establish clinical policies, procedures, and standards of care.
- Education: Master's degree in Nursing (MSN) or Doctoral degree in Nursing (DNP).
- Experience: Minimum 5 years of experience as a Nurse Practitioner in a geriatric care setting.
- Skills: Strong leadership, communication, and interpersonal skills; ability to work collaboratively with multidisciplinary teams.
$145,000 per year, based on experience and qualifications.
