Speech Language Pathologist

VitalCaring Group is seeking a highly skilled Speech Language Pathologist to join our team of healthcare professionals. As a Speech Language Pathologist, you will play a vital role in providing therapeutic techniques for patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders.
Responsibilities:- Provide coordinated care to patients of all age groups, including children, adults, and seniors.
- Plan, implement, and evaluate patient care plans to restore or maintain patient well-being.
- Participate in the coordination of care with other healthcare professionals, including physicians, nurses, and therapists.
- Perform ongoing clinical assessments, including OASIS assessments, to identify patient needs and determine level of functioning.
- Develop and evaluate patient care plans in partnership with patients, representatives, and caregivers.
- Prepare and complete clinical notes, phone orders, and other clinical record documentation in a timely manner.
- Coordinate care and document coordination with all physicians and other healthcare clinicians involved in the care of the patient.
- Provide therapeutic treatments for speech, language, cognitive, and swallowing disorders, as ordered by physicians.
- Document, prepare, and complete clinical notes, verbal orders, and other clinical record documentation in a timely manner.
- Apply concepts of infection control and universal precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
- Accept clinical assignments consistent with education and competence to care for patients.
- Provide supervision and guidance to home health aides as appropriate.
- Communicate with all physicians involved in the plan of care and other healthcare practitioners related to the current plan of care.
- Use effective interpersonal relations and communication skills to achieve desirable outcomes.
- Provide patient, caregiver, and family counseling; provide patient and caregiver education.
- Meet mandatory continuing education requirements of the Agency or licensing board.
- Promote the Agency philosophy and administrative policies.
- Demonstrate commitment, professional growth, and competency.
- Participate in the Agency's QAPI program.
- Participate in Agency-sponsored in-service training.
- Master's or Doctoral Degree in Speech-Language Pathology from an accredited college or university.
- Current State License in Speech-Language Pathology or meets the requirements listed in 42 CFR § n).
- Prefer one year experience in an acute care, rehabilitation setting, or home health.
- Therapy skills as defined by generally accepted standards of practice, good interpersonal skills, and proof of current CPR.
- Reliable transportation, current Driver's License, and valid and current auto liability insurance.
