Key Responsibilities:- Deliver quality patient care and perform technical skilled care in the patient's home, adhering to infection control procedures.
- Continuously observe and report patient nursing needs to supervising RN.
- Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs.
- Document patient visits thoroughly and maintain an accurate medication listing.
- Participate in the performance improvement plan to reduce patient hospitalizations and improve outcomes.
- Attend meetings, including case conferences, and maintain a positive attitude towards learning new skills.
- Schedule flexibility is required to ensure patient care is provided as needed.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
