Job Summary

We are seeking a highly skilled Registered Nurse to join our team at the Robert J. Dole VA Medical Center in Salina, KS. As a member of our Patient Aligned Care Team (PACT), you will provide direct care and care coordination to our nation's Veterans, working collaboratively with a multidisciplinary team to deliver high-quality, patient-centered care.

Responsibilities
  • Provide direct care and care coordination to Veterans, including health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
  • Manage assigned patient panels, utilize registries, and ensure appropriate evaluation and access is provided to patient panels.
  • Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
  • Provide peers with formal or informal constructive feedback for improvement.
  • Support colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care.
  • Foster a safe and supportive environment conducive to the professional development of health care professionals.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • Ability to work effectively in stressful situations, able to shift priorities based on patient needs.
