Job Title: IDD Qualified Professional

Community Care of North Carolina Inc is seeking a highly skilled IDD Qualified Professional to join our team. As an IDD Qualified Professional, you will play a critical role in ensuring the development, supervision, monitoring, revision, and updates of the member's Person Centered Plan/Individual Service Plan.

Key Responsibilities:

  • Develop, monitor, revise, and oversee the member's person-centered plan/individual service plan to ensure alignment with their needs and goals.
  • Supervise, integrate, and coordinate person-centered plans/individual service plans, monitoring progress towards personal, clinical, and functional outcomes.
  • Conduct admission/intake paperwork for new members, including completing forms, referrals, authorization requests, and NCSNAP.
  • Coordinate with Tailored Care Management/Care Coordination from collateral agencies to ensure seamless service delivery.
  • Provide Tailored Care Management/Care Coordination in the absence of agency, ensuring continuity of care.
  • Maintain and manage up-to-date medical records, including PCP/ISP, NC SNAPS, authorizations, primary care collaborations, and CCNC.
  • Obtain input from the person/providers/significant others about the service delivery process and seek information to obtain needed services/supports on behalf of the person.
  • Inform significant others about the person's situation and the care manager's efforts on behalf of the person with their consent.
  • Locate and coordinate sources of help to ensure the individual receives available natural and community supports.
  • Coordinate, arrange, link referrals, and support services necessary for members' care and identified needs.
  • Complete application forms to assist in receiving community and other formal service support.
  • Oversee the facilitation of the service delivery process, from intake/initial assessment to the annual re-evaluation of the individual's needs and services.
  • Monitor the individual's situation to ensure quality care and the continued appropriateness of services.
  • Provide assistance in supervising direct care staff and hold monthly supervisions to address progress/issues related to the treatment of the member.
  • Review documentation for direct care staff and monitor service approval and unit allocation.
  • Perform other duties as required and directed by the IDD Director and/or designee.

Requirements:

  • Bachelor's degree in Social Work, Social Services, Human Services, or Special Education required; or a Master's Degree.
  • Minimum 2 years' experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with a Master's degree.
