About the Role

We are seeking an experienced Advanced Heart Disease Nurse Coordinator to join our team at Mass General Brigham.

Job Summary

The successful candidate will be responsible for coordinating the care of advanced heart disease patients, working closely with multidisciplinary teams, and providing education to patients and families.

Key Responsibilities
  • Coordinate evaluation of advanced heart failure patients being considered for mechanical circulatory support
  • Collaborate with unit staff, MCS nurse practitioners, and physicians to facilitate a comprehensive plan of care
  • Maintain continuous communication with community agencies regarding ongoing care and support
Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts
  • Prior experience (3+ years minimum) with cardiology / MCS-focused patient care experience required
  • Recent experience (within 2 years) in ambulatory nursing strongly preferred
