About the Role

We are seeking a skilled Nurse Practitioner to join our team at Monogram Health, a next-generation, value-based chronic condition risk provider. As a key member of our integrated Care Team, you will play a vital role in delivering personalized, compassionate medical care to individuals with chronic kidney disease and related metabolic disorders.

Responsibilities
  • Conduct comprehensive annual wellness exams and provide counseling and education to patients and families about benefits and programs available to help them live healthier lives.
  • Document patient information, including chief complaint, diagnosis, past medical history, and plan of care.
  • Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians to deliver evidence-based, timely care.
  • Participate in integrated care team meetings and contribute to the development of treatment plans that prioritize patient health and satisfaction.
Requirements
  • Bilingual (English/Spanish) highly preferred.
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and unrestricted DEA certificate.
  • Ability to work autonomously and practice without direct supervision.
  • Access to transportation, a valid driver's license, and car insurance.
  • Proficiency with medical instruments and equipment required by the work.
  • Knowledge of computer-based data management programs and information systems.
