Job Summary

We are seeking a skilled Nurse Practitioner to join our team in a Long Term Care setting in Claremont, NH. The ideal candidate will have 2+ years of current experience and be willing to float to different units as needed.

Key Responsibilities
  • Observe and assess residents on a continuing basis, identifying changes in mental and physical well-being and reporting to the Supervisor as necessary.
  • Administer medications and treatments as ordered by the physician, accurately documenting via a computerized system.
  • Supervise Licensed Nursing Assistants (LNAs) in accordance with licensing requirements and facility policies.
  • Assign and measure nursing staff resident care, supervising the care given and assisting direct-care staff as needed.
  • Accompany physicians on resident rounds to address medical needs and facilitate communication.
  • Perform regular duties on the unit, including checking appointments, monitoring LNA documentation, and maintaining a clean and hazard-free environment.
  • Report problems to the Nurse Manager/Nursing Supervisor and encourage LNA observations and documentation.
  • Admit, discharge, and transfer residents, completing necessary documentation and notifications.
  • Communicate with residents' families to resolve concerns and problems, notifying them of new orders and obtaining consent when necessary.
  • Assume responsibility for the management of assigned units during shifts, following State and Federal Regulations regarding controlled medications.
  • Assist with the orientation of new nursing employees and communicate with the Director of Nursing Services regarding continuing education needs.
Requirements
  • 2+ years of current experience as a Nurse Practitioner in a Long Term Care setting.
  • Willingness to float to different units as needed.
  • IV certification highly preferred.
  • Previous travel experience and rural/small facility experience highly preferred.
