About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in homecare. As a key member of our care team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Perform patient assessments and develop comprehensive care plans in collaboration with the care team.
  • Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
  • Promote patient health and independence through teaching and rehabilitative measures, assisting patients in learning self-care techniques.
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Complete documentation timely and accurately, and regularly communicate patient progress to the clinical manager and care team.
  • Plan and provide staff education, and participate in clinical development and continuing education programs.
Requirements
  • One (1+) year of clinical experience as a Registered Nurse (RN).
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
