We are open to considering a Physical Therapist (PT) as well.
We are looking for a committed Physical Therapy Assistant (PTA) or Physical Therapist (PT) to be part of our exceptional team at Avamere Crestview of Portland. Our team is devoted to delivering high-quality care, utilizing evidence-based practices, and achieving remarkable patient outcomes.

Key Responsibilities:
- Implement and execute established patient treatment plans under the supervision of a licensed Physical Therapist
- Communicate effectively with patients, families, referring physicians, and other team members
- Provide timely documentation in accordance with facility and departmental standards
- Enhance clinical and professional skills through ongoing education and professional associations
- Ensure timely documentation for insurance providers to meet licensing, certification, and regulatory requirements
- Support and participate in departmental operations and development
- Report any changes in patient status or needs to the supervising therapist promptly
Qualifications:
- Graduate of an accredited Physical Therapy Assistant program
- Able to practice without restrictions
- In good standing with all regulatory agencies and licensing boards
- Familiarity with Medicare and other payer systems
- Comprehensive understanding of residents' rights
- Professional demeanor in presentation
- Ability to manage multiple tasks effectively
- Proficient in reading, writing, speaking, understanding, and communicating in English
- CPR certification required
