Key Responsibilities

  • Provide professional and compassionate care to patients in their homes, utilizing all available resources to manage their medical needs.
  • Work collaboratively with other disciplines to maximize patient outcomes, serving as a patient advocate and communicating effectively with the primary Doctor, Clinical Team Manager, and other team members to manage patient care.
  • Develop and implement individualized care plans, taking into account patient preferences and needs.
  • Conduct thorough assessments, identify areas for improvement, and implement changes as needed to ensure high-quality patient care.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.

Requirements

  • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
  • 1 year nursing experience preferred.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment.
