Job Summary

The Ambulatory Care Pharmacist is a key member of the Trinity Health team, responsible for optimizing medication use for outpatients and assisting with transitions of care for hospital discharges.

Key Responsibilities
  • Perform patient interviews and review medical records to execute comprehensive medication assessments
  • Create and execute treatment plans through provider communication, patient education, and care coordination modalities
  • Deliver high-quality, evidence-based, cost-effective pharmaceutical care
  • Collaborate with interdisciplinary teams to optimize medication use and patient outcomes
Requirements
  • Active Pharmacist License and ND Registered Pharmacist license
  • Board Certification in Ambulatory Care within 18 months of hire
  • Bachelor's of Science Pharmacy degree or Doctorate of Pharmacy degree from an accredited school of pharmacy
  • Minimum of two years of full-time experience in relevant pharmacy practice
  • Successful completion of an American Society of Health-System Pharmacists accredited pharmacy residency preferred
Preferred Qualifications
  • Ambulatory Care and Anticoagulation management service experience
  • Interest in teaching pharmacy students and residents
Skills and Abilities
  • Comprehensive knowledge of common disease states and pathologies
  • Ability to apply current pharmacologic and biopharmaceutical principles
  • Excellent analytical, critical thinking, and problem-solving skills
  • Excellent communication, organizational, and computer skills
