Key Responsibilities- Diabetes Self-Management Education (DSME): Provide individualized DSME/training and medical nutrition therapy integrated with patient medical goals to individuals and groups.
- Collaboration and Communication: Work cooperatively with healthcare teams to actively promote holistic care for patients and families.
- Education and Training: Develop and deliver interactive, collaborative, skill-based training methods to maximize patient engagement and understanding.
- Program Management: Collaborate with team members to develop and implement DSME programs, ensuring effective care management services and continuous quality improvement.
- Documentation and Evaluation: Document assessment data, educational plans, care services provided, and evaluation results in patient records.
- Certification: Certified Diabetes Care and Education Specialist (CDCES) in good standing with the Certification Board for Diabetes Care & Education (CBDCE).
- Experience: At least three years of experience in diabetes care, education, and program management.
- Skills: Knowledge of insulin pumps, continuous glucose monitoring technology, and health insurance coverage processes.
