Job Details

We are seeking a BSN-prepared RN to work in the role of Hospice Nurse, covering the greater New Braunfels, TX metro area. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a team environment.

  • Responsibilities:
    • Respond to patients' and families' hospice needs.
    • Perform skilled nursing assessments of physical, emotional, psychosocial, and spiritual needs.
    • Assess and document changes in patients' conditions, evaluating and reporting to the patient's physician and/or Medical Director.
    • Provide bedside nursing, perform nursing procedures, or administer medications and treatments as ordered.
    • Evaluate patients' medication needs for palliation, consulting with the patient's physician/Medical Director.
    • Cooperate in the patient's recertification process, evaluating and documenting the patient's medical condition and decline.
    • Assist families and caregivers at the time of death, handling necessary arrangements such as pronouncing death, preparing the body, arranging for funeral home removal, notification of authorities, and medication/equipment disposal or removal.
    • Listen carefully to patients and families, timely requesting support from social work and chaplains.
    • Document patient assessments, medications, treatments, and responses concisely, accurately, completely, and timely.
    • Participate in nurses', bereavement, and other meetings and activities.
    • Communicate with IDT members and facility staff members.
    • Coordinate and complete patient admissions.
    • Assure caregiver competence by instructing and verifying understanding of disease process, signs and symptoms of approaching death, equipment, supplies, and medication usage; safety measures, including body mechanics and universal precautions.
    • Evaluate and coordinate needed changes in level of care, including hospitalization, respite care, crisis care, and transfers.
    • Coordinate additional services such as referrals for contracted therapists and medical equipment/supply placement and removal.
    • Monitor caregiver stress and ability to care for the patient, reporting needed changes in the POC to the Interdisciplinary Team.
    • Provide emotional support for patient/family/caregiver.
