About the Role:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Harbors Home Health & Hospice. As a LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
Responsibilities:
- Provide individualized one-on-one care to patients in their homes
- Develop and implement personalized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Administer medications and treatments as prescribed by physicians
- Monitor patient progress and report any changes or concerns to the healthcare team
Requirements:
- Current WA state nursing license
- CPR/BLS certification
- Drivers license
- Dependable insured automobile
- Home health, hospice, acute care, and/or skilled nursing facility experience preferred
