As a Nurse Practitioner on our House Calls team, you'll play a vital role in providing annual clinical assessments to patients in the comfort of their own homes. This proactive approach helps identify and mitigate health risks, ensuring patients receive the best possible care.

Key Responsibilities:
  • Conduct in-home assessments, including medical history reviews, system evaluations, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as needed).
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform the patient's primary care physician of potential care gaps.
  • Intervene in urgent and emergent situations as needed.
  • Educate patients on disease processes, medication, and compliance.
Requirements:
  • Active NP license or ability to obtain by start date.
  • Unencumbered, free of any open/unresolved disciplinary actions.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • May be requested to obtain additional licensure in other geographic areas.
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home Health care or home visit experience.
