We are seeking a skilled and compassionate Caregiving Professional to join our team at The Laurels of Hamilton. As an integral part of our care delivery team, you will play a vital role in ensuring the well-being and satisfaction of our residents.

Job Summary

The Caregiving Professional will be responsible for conducting assessments and care plan coordination for assigned residents. This includes completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) within regulated time frames.

You will also assess resident needs through physical evaluation, interview, and chart review, discussing these needs with caregivers and other healthcare professionals as necessary.

Responsibilities
  • Conduct resident assessments and develop individualized care plans that meet their unique needs and preferences.
  • Collaborate with interdisciplinary teams, including physicians, nurses, social services, therapy, dietary, and activity staff, to ensure comprehensive care delivery.
  • Maintain accurate and up-to-date records of resident care, including MDS and CAA completion, care plan updates, and significant change documentation.
  • Participate in Plan of Care (POC) meetings and contribute to resident-centered decision-making.
Requirements
  • Degree in Nursing or related field; RN licensure strongly preferred.
  • Experience working with the Resident Assessment Instrument (RAI) process, Prospective Payment Process (PPS), and electronic health records (EHRs).
  • Ability to communicate effectively with residents, families, and healthcare teams.
  • Strong problem-solving skills, critical thinking, and attention to detail.
