Position Summary:
We are in search of a qualified Full-Time or PRN - Physical Therapist for our Southside office location. The role involves providing physical therapy services in designated areas.

Key Responsibilities:
The Physical Therapist will be accountable for executing care standards for physical therapy services. This includes:

  • Conducting initial assessments and routine follow-ups.
  • Communicating patient-related information back to the office and ensuring accurate documentation.
  • Delivering physical therapy services in accordance with a physician's prescribed care plan.
  • Evaluating therapeutic and rehabilitative needs, including assessing home environments.
  • Instructing patients and their families on the use and maintenance of therapeutic devices.
  • Supervising physical therapy assistants and home health aides involved in patient rehabilitation.
  • Participating in discharge planning and preparing clinical progress summaries.

Qualifications:
Applicants must hold a current Physical Therapy License in the State and preferably have two years of experience in home health settings. Strong communication and organizational skills are essential, along with proficiency in computer applications.

Education:
A Master's degree is preferred.

Certifications:
CPR Certification is required.

Work Schedule:
This position offers a flexible schedule, primarily Monday to Friday.

Compensation:
Competitive pay starting from $38.00 per hour.

Additional Information:
Applicants should be prepared to relocate as necessary.
