Job Summary

Sage Health is seeking a highly skilled Primary Care Physician to join our team in Waverly/Baltimore. As a Primary Care Physician, you will be responsible for providing high-quality patient care and working collaboratively with our care team to deliver exceptional patient outcomes.

Key Responsibilities
  • Evaluate and Treat Patients: Conduct thorough evaluations and provide treatment to patients in accordance with standards of care.
  • Coordinate Patient Services: Assist in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
  • Participate in Daily Huddles: Act as an active participant and key source of medical expertise with the care team through daily huddles.
  • Lead by Example: Help Lead Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
  • Complete Medical Record Documentation: Complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation.
  • Follow Policy and Protocol: Follow policy and protocol defined by Clinical Leadership.
  • Meet with Lead Physician: Meet with Lead Physician about quality of care, review of outcome data, and medical records issues.
  • Clinical Focus: Clinically focused on direct patient care, inclusive of patient-facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care.
Requirements
  • Medical Degree: Graduate of accredited MD or DO program of an accredited university.
  • Medical License: Current and unrestricted medical license or willing to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
  • Board Certification: Board Certification or Eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine preferred.
  • DEA License: Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Number: Medicare Provider Number and Medicaid Provider Number.
  • Interpersonal Skills: Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
  • Team-Based Care: Fully engaged in the concept of team-based care model.
  • Value-Based Care: Willingness and ability to learn/adapt to practice in a value-based care setting.
  • Patient Service: Superior patient/customer service.
  • Computer Skills: Basic computer skills, including email and EMR.
Preferred Qualifications
  • HEDIS/STARS Knowledge: Knowledge of HEDIS/STARS.
  • CAHPS and HOS Knowledge: Knowledge of CAHPS and HOS.
  • Medically Underserved Communities Experience: Experience working in medically underserved/culturally diverse communities.
  • Electronic Health Record Experience: Experience working with an electronic health record.
  • Medicare Advantage Global Risk Models Experience: Experience working in Medicare Advantage Global Risk Models.
