Key Responsibilities:
  • Collaborate with the primary care team to provide patient-specific medical care
  • Provide palliative care services to nursing center residents with serious illnesses
  • Emphasize pain and symptom management, psychosocial support, and advance care planning
  • Serve as a specialized consultant within the interprofessional team
  • Provide guidance and education to patients, families, and healthcare teams
Requirements:
  • Graduate from an accredited School of Nursing program
  • Licensed as a registered nurse and certified as a Nurse Practitioner
  • Minimum of two years experience working with the adult or geriatric population
  • Palliative care certification preferred
