We are seeking a highly skilled Advanced Practice Provider to join our team at LiveWell Choice, LLC. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to our members in nursing home settings.
Job Summary:The Advanced Practice Provider will be responsible for:
- Providing collaborative primary care and services to a panel of Longevity Health members
- Executing and delivering Longevity Health Model of Care activities
- Developing and implementing advanced care planning
- Facilitating health plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS, and STARs quality gap closure
- Serving as a trusted advisor to members, loved ones, and nursing home staff
- Delivering effective and frequent communication and coordination with members' PCPs, loved ones, and care team members
To be successful in this role, you will need:
- A Master of Science in Nursing as a Nurse Practitioner from an nationally accredited program
- Minimum of 6 months experience as a Nurse Practitioner/Physician's Assistant
- Experience working in the nursing home environment is strongly desired
- Computer/Technology proficiency required
- The ability to engage and communicate effectively with physicians, patients, family members - excellent people skills
