Job Title: Registered Nurse - Integrated Care Clinic

ACI Federal Defunct is seeking a compassionate and caring Registered Nurse to join our team at the Integrated Care Clinic. As a Registered Nurse, you will be responsible for providing nursing services for the Integrated Care Clinic and other programs, as assigned. Your duties will include providing direct psychiatric and general medical nursing care for patients, as well as daily nursing oversight of Licensed Practical Nurses.

Responsibilities:
  • Participate in the efficient provision of daily nursing service delivery in the Integrated Care Clinic.
  • Provide psychiatric and general medical nursing services, including medication administration, injections, phlebotomy, CLIA-waived testing, vital sign determinations, AIMS testing, mental status examinations, medication delivery, psychiatric triage, and other assessments as required.
  • Oversee medical protocols relating to the prescribing of clozapine and coordinate blood work with designated laboratories.
  • Communicate education, observations, significant data, and interventions to treatment team members via documentation and other methods.
  • Provide preventive healthcare education for chronic diseases, including diabetes, obesity, hypertension, heart disease, mental illness, and other conditions.
  • Participate in peer record reviews to determine compliance with record-keeping requirements and standards of care.
  • Participate in the ordering, receipt, storage, dispensing, security, and recordkeeping of medications.
  • Assure the security and safety of the Medication Room and its contents.
  • Inventory and requisition needed supplies for nursing tasks.
  • Participate in program team meetings and other required conferences.
  • Provide education to patients individually and within group settings related to proactive and holistic needs in a wellness and recovery-modeled format.
  • Collaborate with the interdisciplinary treatment team and community providers on an ongoing basis to facilitate integrated care.
  • Complete lab slips and call-in prescriptions to pharmacies as authorized by I-Care physicians.
  • Consult with physicians and complete pre-authorization forms when required by insurance companies.
  • Notify patient's physician whenever they report side effects and/or have other serious symptoms.
  • Provide crisis counseling as needed.
  • Comply with established legislation, mandates, clinic standards, and requirements regarding patient services and documentation.
  • Orient new employees to medical and nursing policies and procedures.
  • Participate in staff development activities and in-service training.
  • Act as liaison between the physicians, nursing staff, and other agency staff.
  • Complete required training and certifications by their due date.
  • Perform other related duties as required.
Working Conditions:

Work is performed in clinical and office environments. Requires managing a number of competing work priorities in a time-restricted environment. Works with a challenging population of patients, often from diverse backgrounds, with varying degrees of life stress and comorbid medical and psychiatric conditions.

Requirements:
  • Current and unrestricted Virginia Registered Nurse license (or licensed in one of the Nurse Licensure Compact States).
  • BSN preferred.
  • Minimum of one year of experience working with patients with severe mental illness, co-occurring substance use disorders, and co-morbid general medical conditions, preferably in an integrated psychiatric and primary care outpatient setting.
  • Possession of a valid driver's license and an acceptable driving record as reported by the DMV are required for persons using personal or agency vehicles in the performance of their duties.
  • Must be certified or certifiable in Basic Life Support and First Aid training.
