Licenses/Certifications:
Graduation from an accredited pharmacy institution recognized by a state accrediting body.
Possession of a current, active, and unrestricted pharmacist license from any of the 50 US states, the Commonwealth of Puerto Rico, or the District of Columbia.
Current Basic Life Support (BLS) certification.Duration: 12 Months
Work Schedule:
Monday to Friday, 8:00 AM to 4:30 PM
Position Overview:
The pharmacist will be integral in ensuring prescription accuracy and providing patient education at Prime Physicians, delivering exceptional pharmaceutical care.
Key Responsibilities:- Reviewing medication orders from prescriptions and processing them within the Electronic Health Record (EHR), ensuring thoroughness and appropriateness while checking for allergies.
- Confirming that all prescribed medications are included in the Unit formulary and assessing for potential drug-drug and drug-food interactions.
- Functioning independently and potentially being the sole pharmacist during specific shifts.
- Inputting prescriptions into the pharmacy software system, accurately dispensing medications, and advising patients on their prescriptions.
- Evaluating medication orders for thoroughness, appropriateness, and possible interactions, and clarifying any discrepancies with prescribers as needed.
- Collaborating with other healthcare professionals within the clinic to resolve issues related to medication ordering and delivery.
- Maintaining comprehensive records, particularly for controlled substances.
- Graduation Certificate from an Accredited Pharmacy School.
- A minimum of 2 years of experience as a pharmacist within the last 36 months in the United States.
- Current BLS certification.
- Active Pharmacist License.
