Job Summary

Responsibilities:

  • Provide high-quality physical therapy services to patients, including assessment, diagnosis, and treatment planning.
  • Document patient progress and maintain accurate records according to approved medical and facility procedures.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Discharge patients when goals have been met or medical necessity no longer warrants intervention.

Requirements:

  • 6 months of PT experience within the last 5 years
  • Demonstrated expertise in skilled nursing facility settings
  • Strong communication and interpersonal skills
