About the Role

As a Physical Therapist at Solaris HealthCare Forest Lake, you will be part of a dedicated team of professionals committed to supporting and uplifting our residents. Your primary responsibility will be to provide personalized physical therapy services to our residents, helping them to achieve their full potential and improve their quality of life.

Responsibilities
  • Develop and implement individualized physical therapy plans for residents
  • Conduct thorough assessments and evaluations to identify residents' physical therapy needs
  • Provide hands-on physical therapy interventions, including exercises, stretches, and modalities
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document resident progress and adjust treatment plans as needed
Requirements
  • Successful completion of a physical therapy program approved by the Commission on Recognition of Postsecondary Accreditation or the United States Department of Education
  • Licensure to practice as a physical therapist in the state of Florida
  • Prior experience in physical therapy, preferred
  • Member of the American Physical Therapy Association, preferred
