Job SummaryThe ideal candidate will have a passion for empowering individuals with developmental disabilities to achieve independence and participate fully in their communities. You will work closely with clients, families, and other support staff to develop personalized plans that meet each individual's unique needs and goals.
Key Responsibilities:
- Implement independent living skill curriculum in areas such as healthy meal preparation, hygiene, communication, and community engagement.
- Provide close supervision and training to participants to promote their skills and abilities.
- Offer direct care services, including personal care assistance, home health aide support, and transportation, as needed.
- Maintain a safe and clean living environment for participants, minimizing risks to their well-being.
- Document participants' activities, medication intake, and progress daily, and submit complete and accurate updates to Case Managers in a timely manner.
- Perform administrative tasks, such as filing and data entry, to support the efficient operation of the CFS program.
- Comply with HIPAA regulations and maintain confidentiality in all interactions with clients and their families.
To be successful in this role, you will need:
- A high school diploma or equivalent.
- One year or more experience working with individuals with developmental disabilities.
- A valid California driver's license, current registration, and vehicle insurance.
- Fluent English language skills, both written and verbal.
- Ability to work independently, demonstrate good judgment, and prioritize tasks effectively.
- Strong interpersonal skills and ability to interact positively with clients, families, and other stakeholders.
- Willingness to work flexible hours, including evenings and weekends, to meet client needs.
