Eyesight Specialist

Position Overview

As an independent optometrist leasing space inside Walmart stores, you will play a key role in patient care, providing eye and vision care as well as general health and well-being services.

Key responsibilities include:

1. 
   
2. Establishing your days of practice and hours of operation
   
3. Controlling your patient schedule and setting your fee schedule
   
4. Providing high-quality eye care services, including exams and professional fees
   
5. Opting into equipment, instrumentation, administrative, and/or Opt-Tech support services (if state allows)
   

Minimum qualifications for a lease agreement include:

1. 
   
2. A Doctor of Optometry (OD) degree from an accredited optometry school
   
3. A valid state optometry license
   
4. Ophthalmologist (MD or DO) degree from an accredited school
   
5. A valid state ophthalmology license
   
6. General Liability Certificate
   
7. Professional Liability Insurance Certificate
   
8. A W-9
