Role Summary:
The Advanced Practice Nurse will play a crucial role in supporting the healthcare needs of individuals transitioning from military service. This position involves conducting thorough evaluations and assessments for clients seeking medical benefits.
Key Responsibilities:
- Conduct face-to-face assessments to complete necessary documentation for eligibility determination.
- Ensure accurate and timely submission of all required paperwork.
- Deliver high-quality patient care and maintain exceptional standards during all interactions.
- Collaborate effectively with multidisciplinary teams to enhance service delivery.
- Utilize clinical expertise to gather and analyze patient data, fostering strong relationships with clients.
Qualifications:
- Master's degree in Nursing (Required)
- Certification in Family or Adult Nursing (Required)
- Active Nurse Practitioner License in the state of practice
- Preferred experience in primary care or related fields
- Experience working with veteran populations is a plus
Technical Skills:
- Proficiency in Electronic Health Records systems
- Familiarity with Microsoft Office Suite
Essential Skills:
- Strong assessment and problem-solving abilities
- Excellent communication skills, both written and verbal
- Ability to work independently and manage multiple priorities
- Adaptability to a fast-paced environment
