We are seeking a compassionate and reliable Care Coordinator to join our team at Karing Hearts, LLC. In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client receives high-quality, individualized care.

Key Responsibilities
  • Care Planning and Coordination
    • Collaborate with physicians, patients, families, and healthcare staff to develop and review individualized care plans.
    • Create patient goals and monitor progress to ensure the best possible outcomes.
    • Coordinate care internally and with external providers to ensure seamless transitions.
  • Staff Management and Development
    • Onboard and train new staff members to ensure they have the necessary skills and knowledge to provide exceptional care.
    • Provide ongoing training and management for caregivers to ensure they meet the highest standards of care.
  • Scheduling and Operations
    • Coordinate with schedulers to meet client needs and ensure timely care.
    • Receive and triage inbound calls to address client concerns and provide support.
  • Compliance and Documentation
    • Ensure compliance with all company policies, state, and federal regulations to maintain the highest standards of care.
    • Manage client and caregiver files and credentials in accordance with policies and regulations.
  • Client Relations
    • Handle client inquiries and concerns in a timely and compassionate manner.
    • Conduct regular check-ins with clients and their families to ensure their needs are being met.
  • Quality Assurance
    • Monitor the quality of care provided and implement improvements as needed to ensure the best possible outcomes.
    • Update and train staff on care plans when discrepancies are identified.
Requirements
  • Previous experience as a Care Coordinator or in a similar position is preferred.
  • Previous experience in home care is required.
  • Strong problem-solving and organizational skills to manage multiple projects and prioritize tasks effectively.
  • Ability to work in fast-paced situations and make sound decisions quickly.
  • Excellent interpersonal skills and a high level of compassion to provide exceptional care.
  • Strong verbal and written communication skills to effectively communicate with clients, families, and healthcare staff.
