Job Title: Full-Time Hospice RN Case Manager

At Heartland Hospice, we are seeking a compassionate and experienced RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families.

Responsibilities:
  • Provide direct patient care services and supportive care to patients and their families.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
Requirements:
  • Compassionate, kind, and empathetic to patients and their families.
  • Comfortable providing care wherever the patient calls home.
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
