Key Responsibilities:
  • Provide strengths/needs/culture discovery assessments, crisis planning, and plan development in partnership with families.
  • Assist parents/guardians/foster parents in improving their skills to optimize the home environment by enhancing support systems and increasing the family's ability to problem solve and advocate for themselves and their children.
  • Ensure all activities of the case management process, from admission to discharge, are completed in a timely and effective manner.
  • Coordinate team meetings with other providers, including clinicians, parent support partners, natural supports, etc.
  • Facilitate integration across systems, such as Child Welfare, Juvenile Justice, and Education.
  • Maintain current and accurate records, including intake, assessment, plan of care, progress notes, and referral logs.
  • Provide supervisor with all assigned reports and update information in a timely manner.
  • Develop, write, implement, evaluate, and revise overall service plan goals, ensuring immediate changes are made as individual needs change.
  • Provide outreach services to individuals in their natural environment to foster and support the case management relationship and ensure engagement in their service planning.
  • Ensure client rights are safeguarded throughout the referral, placement, treatment, review, and discharge processes.
Requirements:
  • Targeted Case Management Certification from the Florida Board is preferred.
  • Bachelor's degree in behavioral science, human services, or a related field is required.
  • At least one year of experience working with children, youth, and families is necessary.
  • Effective communication and comprehension of the English language are essential.
  • Ability to form a mutually respectful and balanced relationship with youth and families.
  • Knowledge of Child Welfare and Foster Care Systems is required.
  • Ability to apply youth and family strengths to inform treatment planning.
  • Flexibility to work with families at their convenience, including evenings, weekends, and holidays.
  • Valid FL Driver's License is required.
  • Vehicle must be safe and dependable, as program youth are transported.
  • Ability and willingness to drive to client homes within a reasonable distance from the office.
  • Excellent communication, organizational, and collaboration skills are necessary.
  • A strong belief in the ability of people to grow and change is essential.
  • Possess the knowledge, skills, and attitude needed to appreciate and encourage diversity and understand the concept of cultural trauma as it applies to individual and family functioning.
