Role Objective
The primary objective of this role is to support the Activities Director in executing a comprehensive activity program that aligns with the needs of residents. This includes aiding in the planning, organization, implementation, and assessment of various recreational, social, intellectual, emotional, and spiritual initiatives, in accordance with facility policies and the residents' care plans.

Key Responsibilities
- Facilitates individual and group activities tailored to the preferences and needs of residents.
- Completes the activity section of resident assessments and revises the program as needed.
- Conducts interviews with residents and/or their families to gather insights on activity preferences.
- Coordinates and assists residents in accessing activity venues within the facility.
- Supports the execution of scheduled activities aimed at enhancing the physical, social, and mental health of residents.
- Records attendance for resident activity programs and ensures that all documented progress notes are detailed and reflective of the services provided, including the residents' responses.
- Monitors resident mood, behavior, and participation in activities, reporting any notable changes or concerns to the Activities Director.
- Assists in delivering in-room activity programs for residents who benefit from personalized visits.
- Plans and organizes offsite excursions and manages transportation logistics as required.
- Collaborates with other departments to ensure seamless execution of daily activities.
- Utilizes resident assessment protocols (RAPs) to determine the necessity of care planning for any concerns related to resident activities.

- May encounter risks such as falls, burns from equipment, and exposure to various odors throughout the day; may experience reactions to dust, tobacco smoke, disinfectants, and other airborne contaminants.
- Potential exposure to infectious materials, diseases, and conditions including AIDS, Coronavirus, Hepatitis B, and Tuberculosis.
- May interact with emotionally charged residents, family members, visitors, or staff.
- Work is conducted in all indoor areas of the facility, with regulated heating and lighting.
- Must be able to stand, walk, and navigate stairs. Occasionally required to move equipment weighing up to 25 pounds.
- Involves bending, stooping, kneeling, crouching, overhead lifting, and other common physical movements as necessary for the role.
- Requires prolonged periods of standing and walking.
- Ability to communicate effectively, write clearly, enter data into a computer, and comprehend written and verbal information.
- Must be able to lift and carry up to 35 pounds frequently; up to 50 pounds occasionally.
- Regularly required to stand and walk, perform simple grasping, fine hand manipulation, and have near and far vision capabilities; must be able to hear conversations and speak clearly.
Qualifications
Minimum Education: High school diploma or GED, or equivalent is required.
Experience: Prior experience in performing essential functions of this role is preferred.
Certifications/Licenses: Certified Nursing Assistant (CNA) in good standing is preferred. CPR certification is also desirable.
