Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, medical history reviews, and provide health education to patients. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure comprehensive care.

Responsibilities
  • Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients
  • Work collaboratively with physicians, case managers, and other healthcare professionals to ensure comprehensive care
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
  • Collaborate with primary care physicians on patient education and provide follow-up care
Requirements
  • Masters degree required or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority
  • Board certified by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with employee health/medical record software
  • Excellent verbal and written communication skills
