Home Health Physical Therapist

We are seeking a dedicated Physical Therapist to join our team at Pathwell Health. As a Physical Therapist, you will play a vital role in assessing, planning, and implementing rehabilitative programs that improve mobility, relieve pain, and increase strength for our clients.
Key Responsibilities- Develop and implement individualized conditioning and rehabilitation plans in partnership with clients, caregivers, and physicians.
- Conduct thorough assessments and screenings to evaluate client physical strength and function.
- Educate and instruct clients and their representatives on exercise programs and therapeutic procedures.
- Accurately document observations, interventions, and evaluations related to client care management and services provided.
- Provide direct patient care as prescribed, including evaluation and treatment planning.
- Review physician referrals and patient medical records to determine diagnosis and required physical therapy treatment.
- Record prognosis, treatment, response, and progress in patient charts or electronic records.
- Identify and document client goals, anticipated progress, and plans for reevaluation.
- Evaluate treatment effects at various stages and adjust treatments to achieve maximum benefit.
- Participate in educational activities and clinical studies as assigned.
- Graduation from an approved Physical Therapy curriculum.
- Current and valid Virginia Physical Therapist license.
- Recent work experience in home health (preferred).
- Strong clinical assessment, organizational, and communication skills.
- Ability to handle multiple assignments simultaneously.
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation.
- Bilingual (Spanish) preferred.
- Strong interpersonal skills and good judgment.
- Ability to work well under pressure and remain calm in emergent situations.
- Self-motivation and a results-driven approach.
- Desire for further education to stay current with medical advances.
