About the Role:

We are seeking a skilled Nutrition Care Specialist to join our team at CommuniCare Family of Companies. As a key member of our dietary services team, you will play a vital role in ensuring the nutritional needs of our residents are met.

Key Responsibilities:

  • Conduct quarterly and annual nutrition assessments on all residents, following the Nutrition Care Process.
  • Develop and implement nutrition interventions, documenting medical and nutrition-related data, providing individualized education, and monitoring daily intakes.
  • Actively participate in quality assurance and performance improvement programs, care plan and nutrition at risk meetings, and openly communicate with other interdisciplinary team members.
  • Monitor quality and accuracy of food served, as well as acceptance of food and beverage by residents during meal rounds.

Requirements:

  • Must be licensed and registered as a Dietitian in the State of employment.
  • Long Term Care/Geriatric nutrition experience is preferred.
