Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health LPN. As a Home Health Nurse for Pinnacle Home Care Inc., you will travel to patients' homes to administer services and provide a variety of tasks during patient visits, depending on the specific plan of care and type of patient.

Responsibilities

  • Provide skilled nursing care under the direct supervision of the Registered Nurse (RN)
  • Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care
  • Responsible and accountable for making decisions that are based on the individual's nursing experience and educational preparation
  • Timely completion of patient documentation

Qualifications

  • Current State LPN or LVN License
  • IV Certification
  • Graduate of an accredited practical nurse or vocational nursing program
  • One (1) year of nursing experience, with community/home health or medical/surgical experience preferred
  • Possess and maintain valid CPR certification
  • Complies with accepted professional standards and practice
  • Demonstrates good verbal and written communication, and organization skills
  • Licensed driver with an automobile that is in good working order and insured per the organization's requirements
