Job Overview

We have a Full Time or Part Time Registered Nurse opportunity open now for Home Health Visits throughout New Castle County.

Job Responsibilities:
  • Deliver high-quality patient care by following designated care plans and reporting changes to Clinical or Client Services Manager.
  • Make home visits to clients in assigned territories, performing assigned duties including administration of medication, wound care, treatments, and procedures.
  • Monitor client conditions, accurately document observations, interventions, and evaluations, utilizing state-of-the-art technology.
  • Case management and coordination with healthcare teams.
Requirements:
  • A current license as a Registered Nurse in Delaware.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is preferred but not required.
  • Ability to work independently and manage time effectively, along with strong interpersonal skills.
  • Solid computer skills, especially with electronic medical records (EMR), and ability to travel to cases as assigned.
